At Allegiance Health Care, we understand that secure housing is more than just a roof—it’s a cornerstone of health, stability, and dignity. Our mission is to guide individuals through challenging housing transitions and help them achieve lasting independence.
Be 18 years or older
Be on Medical Assistance (MA)
Have a documented disability such as:
Physical disability or chronic illness
Mental health condition
Developmental disability
Substance use disorder
Learning disability
History of institutional care with ongoing support needs
Be assessed to need help with at least one of the following due to the disability:
Communication
Mobility
Decision-making
Managing challenging behaviors
Currently homeless
Recently transitioned or transitioning from institutions (hospital, foster care, IRTS, treatment facility, etc.)
At risk of homelessness, such as:
Living in unsafe, temporary, or unstable housing
Facing eviction or utility shutoffs
Lacking a formal lease (e.g., doubled-up housing)
Being discharged from a treatment facility or jail with no place to go
Eligible for one or more of the following waivers:
Brain Injury (BI)
Community Access for Disability Inclusion (CADI)
Community Alternative Care (CAC)
Developmental Disability (DD)
Elderly Waiver (EW)
